Since 1 April 2008 all local safeguarding children boards (LSCBs) are required to review the deaths of all children in their area, as outlined in Working Together to Safeguard Children 2010 (Chapter 7).

The key purpose of reviewing all child deaths is to learn lessons and reduce the incidence of preventable child deaths in the future.

The LSCB data collection is designed to collect information on the number of child deaths where the review of the child death has been completed by one of the Child Death Overview Panels (CDOPs) in England and the number of these cases where the CDOP has assessed that the deaths were preventable.
