The Transition Support Programme (TSP) seeks to raise the standards of transition support and achieve greater consistency in all local areas. The national transition support team (NTST), National Strategies (NS) and Child Health and Maternity Partnerships (CHaMP) are supporting local areas to deliver the TSP. This support includes one-to-one work, project planning, networks and developing good practice and resources.

Transition Support Programme 2010-11

Following completion of a self-assessment questionnaire, all 152 local areas have been assigned to a development stage with scope to progress. Each local authority (LA) area has a named transition advisor. They will be supporting each area to progress to the next development stage.

The spread of local areas between the development stages and funding and support is as follows:

  • Nine local areas are at Development Stage 1 – not fully meeting minimum standards – and will receive 15 days advisor support and £10,000;
  • 103 local areas are at Development Stage 2 – meeting minimum standards – and will receive 10 days advisor support and £25,000;
  • 40 local areas are at Development Stage 3 – working beyond minimum standards – and will receive 10 days advisor support and £50,000.

In addition to the support and funding outlined above, every local area has access to:

  • Support from the 10 SEN Regional Hubs who each have specific funding of £25,000 for regional work on the TSP.
  • Information and guidance – NTST can provide access to specialist advice as well as case studies and ‘How to’ guides.
  • The knowledge and experience from within the TSP partnership. The three partners – NS, CHaMP and NTST – cover knowledge, skills and support in transition across education, health, social care and the voluntary sector.

In addition, over 90 per cent of local areas have successfully bid and received innovation and outcomes funding of £10,000+ to support work up to March 2011.
