For each assessment, the induction tutor completes the induction forms which you can download from this page. The headteacher/principal keeps a copy and sends a copy to the appropriate body within ten days of the NQT completing the assessment period. The original should be retained by the NQT.
Note that these forms may be amended/added to by appropriate bodies for use within their institutions but none of the original fields should be removed or altered.
